Instructions for Completing Your Customer Application:
Please complete the required forms in full and ensure they are signed before submitting. Once ready, send the completed documents to info@janed.net. If applicable, be sure to include a copy of your sales tax-exempt certificate.
Important Payment Information:
- We do not accept American Express. Only VISA and Mastercard are accepted.
- If you plan to pay by credit card, you only need to return the Customer Application Form.
- Please note: All credit card payments are subject to a 3% processing fee.
For Net 30 Terms:
To apply for Net 30 payment terms, you must submit both the Customer Application Form and the Credit Reference Form.
